How we approach medication here
This is not a 15-minute refill mill. The point of the visit is to decide whether the current plan still makes sense.

Diagnosis first, then the prescription
A medication only helps if the diagnosis is right. We start with a full evaluation, not a refill request. If something was missed earlier, that changes what we prescribe.

One change at a time when we can
Stacking new meds on top of a confusing regimen makes it impossible to know what is helping. We simplify when we can, adjust one variable when we cannot, and track what happens.

Side effects are part of the conversation
Weight gain, sexual side effects, brain fog, insomnia. If a medication is working but costing too much in side effects, that is not a success. We say so and look at options.

What the first few months look like

Full evaluation, in person
A long first visit covering your history, every psychiatric medication you have tried, doses, durations, and what actually happened on each one. This appointment has to happen at Rockville or Frederick.

A plan you agreed to
We walk through the options that fit and what your insurance typically covers. We usually cannot check pharmacy pricing for oral medications, since those costs vary by plan and pharmacy. You decide. Nothing gets prescribed without that conversation.

Start and check in
Early follow-ups are closer together so we can catch side effects and decide whether the dose is right. Once things are stable, visits space out.

Ongoing management
Refills, dose adjustments, and check-ins happen on a schedule that matches how you are doing. Established patients can do many of these by telehealth anywhere in Maryland.
What is different about medication management here
Same practice that does Deep TMS and Spravato. Same standards for the everyday prescribing work.

Psychiatrist-led care
Prescribing is done by a board-certified psychiatrist or a nurse practitioner working under physician oversight in the same practice.

One chart for everything
If you later start TMS or Spravato, the same team is looking at the same notes. Medication and advanced treatment are not siloed.

We coordinate with your therapist
If you already have a therapist, we work with them rather than duplicating their work. Medication and therapy do different jobs.

Clear about what we do not treat
We do not treat Bipolar I, psychosis, schizophrenia, substance use disorders, personality disorders, or anyone under 18. Better to know that before you book.
Insurance and medication visits
Initial evaluations and follow-up medication visits are billed as standard outpatient psychiatry under the plans we accept, including Medicare, Maryland Medicaid, and major commercial carriers. Your responsibility is whatever your plan sets for a specialist visit.
We verify benefits before the first appointment so there are no surprises. Controlled substances, when prescribed, follow Maryland and federal requirements.
Medication questions we get asked
Do I need a referral for medication management?
No. Book directly by phone or through the site. We will check your insurance before the first visit.
Can my first medication appointment be virtual?
No. Initial evaluations happen in person at Rockville or Frederick. Once you are an established patient, many follow-ups can be by video anywhere in Maryland.
How long is a medication follow-up?
Long enough to cover how you are doing, side effects, and whether the current plan still fits. We do not run 10-minute refill visits.
What if I have already tried several antidepressants?
That is one of the most common reasons people come here. We review the full trial history, check whether the diagnosis is right, and talk about whether Deep TMS, Spravato, or another medication path makes more sense next.
Do you prescribe stimulants for adult ADHD?
Yes, when clinically appropriate after a proper evaluation. That includes adults who were never assessed as children. Controlled substances follow Maryland and federal rules.
Will you take over my current prescriptions?
After the evaluation, yes, if we agree to continue care. We do not refill medications without establishing you as a patient and reviewing the full history.
Do you coordinate with my therapist?
Yes. If you already have a therapist, we work with them. Medication management here is not a substitute for therapy.
Do you see children or teenagers?
No. We treat adults 18 and older only.
